Services

Juliet Rose Wellness LLC provides telehealth women's health services including hormone therapy, menopause care, sexual health, PCOS management, and general gynecologic wellness, provided by a WHNP-BC licensed in Arizona and Utah. Services available only to patients physically located in these states.

Provider-Patient Relationship

A provider-patient relationship is established only after intake completion, clinical evaluation, and mutual agreement. Browsing this website does not constitute a provider-patient relationship.

Prescribing Practices

All medications, including controlled substances, are prescribed only after appropriate evaluation, testing, and establishment of a legitimate provider-patient relationship. We comply with all DEA and applicable federal and state regulations.

Payment and Cancellation

We are a direct-pay practice accepting credit cards, debit cards, HSA, and FSA. A $100 non-refundable deposit is required to reserve consultation appointments. Cancellations made more than 48 hours in advance receive a refund of the remaining balance. Cancellations within 48 hours are non-refundable. Monthly membership may be cancelled with 30 days written notice.

Limitation of Liability

We are not an emergency service. If you are experiencing a medical emergency, please call 911 or go to your nearest emergency department.

Governing Law

These Terms are governed by the laws of the State of Arizona.
