Concierge-Style
Membership
We offer membership-based health plans for women seeking hormonal care, GYN care, sexual health, and more. We are a cash-based practice and do not participate with any health insurance plans. This allows us to provide the time, attention, and continuity of care that midlife women deserve.
How It Works
From frustrated to thriving —
here's how we get there
No more being shuffled through the system.
Your care begins with a comprehensive visit designed to give us the full picture.
01
New Patient Consultation
A 55-minute comprehensive visit where we take the time to truly understand you. This is the visit you've been waiting for. During this session, we will:
Review your complete medical background, medications, and symptom history
Explore how your symptoms connect to lifestyle and hormonal changes
Order clinically appropriate, customized lab work based on your individual history
Discuss your health goals and begin building your personalized care plan
A new patient consultation is required with or without a membership. If you're unsure about membership, the consultation is a good time to discuss further.

Many patients begin noticing improvements within 2–4 weeks of starting their personalized plan, with more significant changes by 6–8 weeks. Every woman is different, and we set realistic expectations during your consultation while continuously monitoring and adjusting your plan for optimal results.

Juliet Rose Wellness is a direct-pay practice, not in-network with insurance. This is intentional — it allows us to spend 60 minutes with you instead of 12, order the labs you actually need, and provide the continuity of care that delivers results. We accept credit cards, debit cards, HSA, and FSA. Superbills available for potential out-of-network reimbursement.
-
We provide telehealth services to patients physically located in Arizona at the time of their appointment. We plan to expand to Utah in the future.

Most primary care providers run a basic metabolic panel and check TSH. We go deeper — ordering comprehensive hormone panels, metabolic markers, and targeted labs based on your specific symptoms. The goal isn't just to confirm you're "in range" — it's to understand where your levels are relative to where they should be for you to feel your best.
-
All hormone therapy — estrogen, progesterone, and testosterone — is prescribed only after thorough clinical evaluation, comprehensive health history, appropriate laboratory testing, and establishment of a legitimate provider-patient relationship. Prescriptions are individualized, evidence-based, and require ongoing monitoring with regular follow-ups and labs. We do not prescribe hormones without proper clinical evaluation.
-
When prescribed at appropriate physiologic doses and monitored by a qualified provider, testosterone can be safe and effective for women with androgen deficiency. We follow current evidence-based guidelines, use the lowest effective dose, and monitor labs regularly. Testosterone is a Schedule III controlled substance prescribed in compliance with all federal and state regulations.

A $100 non-refundable deposit reserves your consultation. Cancellations 48+ hours in advance receive remaining balance refund. Cancellations within 48 hours are non-refundable. Monthly membership may be cancelled with 30 days written notice.
